The panel found ā€œnumerous mistakesā€Ā and ā€œspecific failuresā€ by the agency and warned that ā€œanother Butler can and will happen againā€ unless ā€œfundamental reformā€œ is enacted.

An independent, bipartisan review identified ā€œnumerous mistakesā€Ā by the Secret Service and ā€œspecific failures and breakdownsā€ that enabled the assassination attempt that injured former President Donald Trump in Butler, Pa., in July.

The panel, made up of four former senior law enforcement and government officials, also warned ofĀ another catastrophic security lapseĀ if the Secret Service does not immediately undertake ā€œfundamental reform.ā€

ā€œThe Secret Service has become bureaucratic, complacent, and static,ā€Ā the panel wrote in a letter to Department of Homeland Security Secretary Alejandro Mayorkas, who overseesĀ the organization.
